WHCP’s services are designed to provide disease prevention resources and education to individuals in the Northern Manhattan community who are at high risk for transmitting HIV and/or HCV due to high-risk drug use or sexual behavior. WHCP provides Syringe Exchange, Low-threshold Case Management and medical care, as well as facilitated access to free HIV testing and follow-up comprehensive care if necessary.
WHCP serves individuals in a wide array of situations however our team is specifically adept at reaching homeless individuals and those who have had negative experiences with the social services system in the past. Our experienced and highly skilled outreach team uses culturally competent and linguistically appropriate means of contact and engagement. WHCP staff, peer educators, interns, and volunteers, are able to build trust and create relationships with at-risk individuals- many of whom are not engaged in any type of medical or mental health care.
We work with individuals to define their goals and use low-threshold case management activities (including but not limited to supportive counseling, pre-referral support and planning, advocacy, and escorts) to ensure access to services and success in reaching their goals- with an emphasis on wellness and recovery.
